Payment is due at the time of services, in the form of cash, credit card, check, Care Credit, or a Scratch Pay loan. Payment plans are not offered in house.

Any checks returned are subject to additional charges.
I acknowledge the cancellation and no show policy, which states that if I do not give 24 hours notice to cancel my appointment, I am subject to a $50 missed appointment fee.
